Extended care

Cogent Thinking’s 24/7 Triage service supports workers from day 1 to day 3 post-injury. Typically, ongoing case management then transitions to the customer’s internal Recover at Work or Injury Management team.

However, Cogent can extend this early-intervention support beyond day 3, continuing to coordinate treatment, communication and return-to-work planning while the case remains in the pre-claim stage.

This extended care program ensures recovery momentum is maintained, supporting workers and their managers with graduated programs towards to return to work goal. Aimed at achieving full return to work in pre-claim stages allows the Recovery at Work or Injury Management teams to focus on the more time consuming or complex accepted Workcover claims.

Recovery momentum is actively supported for return-to-work goals.

How it works

Why Extended Care matters

By supporting workers during the vulnerable pre-claim period, Extended Care helps:

  • Maintain engagement and participation in work
  • Improve recovery outcomes
  • Provide faster access to treatment and medical clarity
  • Ease the burden on internal teams
  • Enable earlier identification and management of risk
  • If the case progresses to claim, workers do so in a positive recovery position

Flexible, customisable support

Extended Care is typically delivered for 2 to 6 weeks, but can be tailored to your operational model and injury profile. If a worker returns to normal duties sooner, the case is closed with appropriate communication and confirmation from key stakeholders.

We co-design implementation with customers to ensure the model integrates seamlessly and remains fit-for-purpose.
